Florida law separates injunctions for protection into five categories: Domestic Violence, Repeat Violence, Dating Violence, Sexual Violence, and Stalking. When determining the type of Injunction for which you need to file, it is very important that you read over all of the requirements for each type before selecting one.

Designation of E-Mail Address Not Represented By an Attorney Pursuant to Fla. R. Gen. Prac. & Jud. Admin. 2.516(b)(1)(D) prose or self-represented litigants are required to provide an email address in which they are a party. Click Here
Request To Be Excused from E-Mail Service This form is to be used for requests to be excused pursuant to Fla. R. Gen. Prac. & Jud. Admin. 2.516(b)(1)(D) from the requirements of e- mail service because you are not represented by an attorney, do not have an e-mail account, or do not have regular access to the internet. Click Here
Notice of Change of Address or E-Mail Address This form is required to keep the clerk’s office and any opposing party or parties notified of current mailing address or e-mail address. Click Here
Notice of Related Cases (Form 12.900(h)) This form is required to indicate if there are any related cases that involve the same parties, children, or issues. Click Here
Notice of Hearing (Form 12.923) Anytime you have set a hearing before a judge, you must send notice of the hearing to the other party. Click Here
Process Service Memorandum You should use this form to give the sheriff’s department (or private process server) instructions for serving the other party in your case and other papers to be served. Click Here

Petitioner’s Request for Confidential Filing of Address (Form 12.980(h)) This form should be filed if you fear that disclosing your address to the Respondent would put you in danger. Click Here
Uniform Child Custody Jurisdiction and Enforcement Act Affidavit (Form 12.902(d)) This form is required if you are asking the Court to determine issues with regard to your parenting plan or time-sharing for a minor child(ren). Click Here
Family Law Financial Affidavit (Short Form) (Form 12.902(b)) This form is required only if you are asking for child support. Use form (b) if your individual gross income is UNDER $50,000 per year. Click Here
Family Law Financial Affidavit (Long Form) (Form 12.902(c)) This form is required only if you are asking for child support. Use form (c) if your individual gross income is $50,000 OR MORE per year. Click Here
Motion for Modification of Injunction for Protection Against Domestic Violence (non-child support/custody/visitation related) For changes (modifications) NOT in regard to child support, custody and/or visitation, either party may request changes to the provision of the Injunction for Protection by filing a Motion for Modification of Injunction for Protection at the Clerk’s Office.
