Passport Information

The Clerk’s Office is a designated passport acceptance facility authorized to accept first-time passport applications, children’s renewals, lost passports, or adult passports expired more than 5 years on behalf of the U.S. Department of State.

If you are traveling in less than 3 weeks, it is strongly recommended that you contact the National Passport Information Center at 1-877-487-2778 or visit Travel.State.gov to make an appointment at a U.S. Department of State Passport Agency.

Passport photos are now available at the Collier County Courthouse location for $15.00 plus tax.

PASSPORT BOOK VS. PASSPORT CARD

  • U.S. Passport Book
  • Required for all international air travel. Valid for travel by air, land, or sea.
  •  
  • U.S. Passport Card
  • Valid for land and sea entry from Canada, Mexico, Caribbean, and Bermuda. NOT valid for international travel by air.

APPLICATION – DS-11 FORM

Each applicant must appear in person regardless of age. Complete all items in black ink pen – DO NOT SIGN. Don’t use whiteout or strike out mistakes. If you have an appointment scheduled, your application MUST be fully completed prior to your visit.

Children

Applicants 16- and 17-Year-Old

If 16- or 17-year-old applicant has a valid driver license, they may apply without a parent present. It is highly recommended that at least one parent appears, with valid, physical photo ID, as proof of parental awareness & consent. Applicants Under 16 Years old

Both parents or guardians must appear with valid, physical photo ID. If not, form DS-3053 must be completed by the absent parent or guardian.

Evidence of the child’s relationship to parents/guardian(s).

Documentation must be submitted that lists the parent(s) or legal guardian(s) of the child. Some documents, like a U.S. birth certificate, show both U.S. citizenship and parental relationship.

Examples include:

  • U.S. birth certificate (also evidence of U.S. citizenship)
  • Foreign birth certificate
  • Adoption decree
  • Divorce or custody decree
  • Court order

Children born outside the U.S. or for other special circumstance, call our office or visit Travel.State.Gov.

DOCUMENTATION – REQUIRES 2 TYPES

Proof of citizenship

YOU MUST submit an original document or certified copy. This document will be mailed to the U.S. Department of State and returned once your passport has been issued.

Examples of acceptable documents:

  • U.S. Birth Certificate
  • Undamaged U.S. Passport (expired accepted)
  • Certificate of Naturalization
  • Certificate of Citizenship
  • Consular Report of Birth Abroad or Certification of Birth

Government Issued ID

MUST physically present an acceptable form of ID. If out-of-state, must present additional ID. For a full list of acceptable primary and/or secondary identifications please visit Travel.State.Gov.

Primary ID examples:

  • Fully valid Florida Driver’s License
  • Valid or expired, undamaged U.S. Passport
  • Certificate of Naturalization
  • Current (valid) foreign passport
  • U.S. Permanent resident card
  • In-state, fully valid non-driver ID with photo

COPIES

A copy of the Proof of Citizenship document AND the Government Issued ID is required.

Copies must be black and white, on 8 ½ inch by 11-inch paper, single sided and include the front and back of each document. Copies are available in our office for $0.15 per page.

PHOTO

Submit one photo with each application.

Requirements:

  • 1 – 2” x 2” color photo, white background, taken within last 6 months.
  • NO eyeglasses, head covering or uniforms.
